Complete enrollment before membership care begins
Serenity membership benefits do not begin, and the initial membership appointment cannot be scheduled, until all required enrollment steps are complete.

Sign Your Agreement
Complete and electronically sign the applicable Serenity Membership Agreement.

Make Your First Payment
Submit the first scheduled payment for the membership option you selected.

Submit Your Deposit
A membership deposit equal to one month of the selected membership rate is due at enrollment.

Authorize Recurring Payment
Establish the required automatic recurring ACH or card payment method for your membership.
After all enrollment requirements are complete, you can proceed with scheduling your initial Serenity membership appointment.

One-Time Non-Member Psychiatric Consultation
Patients who are not currently enrolling in Serenity may schedule a one-time psychiatric consultation at the non-member rate.
This consultation is separate from Serenity membership enrollment.

When does my membership become active?
